Neuromechanical coupling during mechanical ventilation: reverse-triggering vs others forms of asynchrony

چکیده
Introduction The term “respiratory entrainment” refers to of coupling patterns between the rhythm neuronal and mechanical of ventilator to achieve satisfactory synchrony. Respiratory effort stimulated by the mechanical inspiratory flow or reverse trigger (RT), has been recently demonstrated [1]. This asynchrony is difficult to showed without additional signals as esophageal pressure or electrodiaphragmatic signal.

A randomized clinical trial of neurally adjusted ventilatory assist versus conventional weaning mode in patients with COPD and prolonged mechanical ventilation
BACKGROUND Patient-ventilator asynchrony is a common problem in mechanically ventilated patients; the problem is especially obvious in COPD. Neutrally adjusted ventilatory assist (NAVA) can improve patient-ventilator asynchrony; however, the effect in COPD patients with prolonged mechanical ventilation is still unknown. The goals of this study are to evaluate the effect of NAVA and conventional...
متن کاملPatient-ventilator asynchrony in a traumatically injured population.
BACKGROUND Prolonged mechanical ventilation, longer hospital stay, and a lower rate of home discharge have been reported with patient-ventilator asynchrony in medical patients. Though commonly encountered, asynchrony is poorly defined within the traumatically injured population. METHODS Mechanically ventilated trauma patients at an urban, level-1 center were enrolled. Breath waveforms were re...
متن کاملMonitoring of patient-ventilator interaction at the bedside.
Monitoring of patient-ventilator interactions at the bedside involves evaluation of patient breathing pattern on ventilator settings. One goal of mechanical ventilation is to have ventilator-assisted breathing coincide with patient breathing. The objectives of this goal are to have patient breath initiation result in ventilator triggering without undue patient effort, to match assisted-breath d...
متن کاملNeurally adjusted ventilatory assist and proportional assist ventilation both improve patient-ventilator interaction
INTRODUCTION The objective was to compare the impact of three assistance levels of different modes of mechanical ventilation; neurally adjusted ventilatory assist (NAVA), proportional assist ventilation (PAV), and pressure support ventilation (PSV) on major features of patient-ventilator interaction. METHODS PSV, NAVA, and PAV were set to obtain a tidal volume (VT) of 6 to 8 ml/kg (PSV₁₀₀, NA...
متن کاملTriggering of the ventilator in patient-ventilator interactions.
With current ventilator triggering design, in initiating ventilator breaths patient effort is only a small fraction of the total effort expended to overcome the inspiratory load. Similarly, advances in ventilator pressure or flow delivery and inspiratory flow termination improve patient effort or inspiratory muscle work during mechanical ventilation. Yet refinements in ventilator design do not ...
